- Parathyroidectomy is definitive for surgical candidates (symptomatic, stone risk, osteoporosis, very high Ca, reduced kidney function, age <50, etc.)
- Non-operative: hydration, moderate calcium intake (not zero), cinacalcet to lower Ca when appropriate, osteoporosis therapy to protect bone
